"

The Registered Nurse provides infusion services for patients at home based on the orders of a physician or other licensed prescriber. This position assists in the coordination of home health services after patients are referred for service", to assist in the transition from hospital to home care and ensure continuity of care between hospital, clinic setting, SNF, physician's office and home. Responsible for continually re-evaluating the patient's nursing needs.

Required Qualifications:

• Completes necessary education to attain RN Licensure in state of California

• California Registered Nurse License upon hire

• National Provider BLS - American Heart Association upon hire

• 1 year Nursing experience in clinical setting

Preferred Qualifications:

• IV and Central lines experience

• Intravenous infusion experience

• Experience with EHR

• Home health field experience

"Providence Home and Community Care (HCC) is a service line within Providence with over 8",000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
